本书详述的新进展似乎普遍明朗化或日益朗化。但这种表面的现象背后隐藏着更为暗淡的力量，使得前景不容乐观。艾滋病的大流行已加快步伐；糖尿病患者正在倍增；全球变暖使新的疾病进入居民免疫力差的地区——这些效应在第9章和第14章描述；面临病原体进化，曾经值得信赖的抗生素变得无效；航空正将全新的疾病（如SARS）扩散到全球；医源性疾病从未像现在这样常见……
